Methylene Blue

Mechanism

A synthetic phenothiazine redox dye that, at low doses, reduces oxidized hemoglobin (Fe3+ methemoglobin) back to functional Fe2+ hemoglobin via NADPH-dependent reduction; it also has reported mitochondrial electron-cycling and monoamine oxidase-inhibitory properties relevant to investigational neuro/metabolic uses.

Half-life

Approximately 5-6.5 hours (terminal half-life) reported for the injectable product.

Regulatory status

FDA-approved as an injection (ProvayBlue, methylene blue injection USP; initial US approval 2016) for pediatric and adult acquired methemoglobinemia. Cognitive, antimicrobial, and longevity uses are not FDA-approved and are off-label/investigational; non-pharmaceutical 'lab grade' products are not for human use.

Safety

Can cause serotonin syndrome when combined with serotonergic drugs (it is an MAO inhibitor) and is contraindicated in G6PD deficiency (hemolysis risk) and pregnancy; may cause blue/green urine and skin discoloration, nausea, and dizziness. High doses can paradoxically cause methemoglobinemia.
